Make a Claim

Claims Guidelines


A) Claim Notification
  • Notification may be made by submitting the relevant claim form, duly completed or any other written notification in the form of email, fax, letters etc.
  • Policy number, NRIC, date of event, type of claim and diagnosis, where applicable, must be indicated.

 

B) Claim Form Requisition
  • The claim form is available at our Head Office, your nearest CIMB Bank or can be downloaded from our website at www.cimbaviva.com.

 

C) Completion of Claim Form
  • All information required in the claim form must be provided and the form duly signed by the Claimant/Life Assured, witness or Medical Attendant accordingly.

 

D) Claim Submission
  • You are advised to submit your claim together with all supporting documents to us as soon as possible.
  • For submission of the claim documents, you may either seek the assistance of your authorised representative, if any, or CIMB Bank’s staff. Alternatively, you may send the claim documents directly to us at our Head Office.
